ABSTRACT

Fundus vascular diseases, including neovascular age-related macular degeneration(nAMD)and diabetic retinopathy(DR), are the leading causes of visual impairment worldwide. With the accelerated aging and increased incidence of diabetes, the prevalence of these two fundus diseases will continue to rise. Currently, intraocular injection of anti-vascular endothelial growth factor(anti-VEGF)remains the first-line treatment for fundus vascular diseases, but disadvantages exist, such as frequent intraocular injections, high cost and poor compliance, thus more durable and effective therapeutic strategies need to be explored. The successful application of gene therapy in inherited retinal diseases(IRDs)provides a new idea for the treatment of fundus vascular diseases. With the ongoing of several clinical trials, gene therapy for fundus vascular diseases is expected to be employed in the clinical setting. But there still remain some concerns, including the optimal therapeutic targets selection, administration route and safety issues. This review focuses on the application and prospect of gene augmentation and gene editing-mediated anti-VEGF therapy for the treatment of nAMD and DR.

ABSTRACT

Objective@#To understand social anxiety and relevant factors among graduate students under the normalization stage of COVID-19 epidemic prevention and control.@*Methods@#Using convenience sampling method, an online questionnaire survey on graduate students from 5 universities in Jiangsu Province was conducted. Measurements used in the survey includes General Self Efficacy Scale (GSES), General Alienation Scale (GAS), Interaction Anxiousness Scale (IAS) and self made survey for basic information and household living conditions.@*Results@#The overall score of graduate students self efficacy was (2.58±0.50). Average score was (30.68±6.22) for alienation, and (47.55±8.77) for interaction anxiety, with detection rate of social anxiety being 43.96%. Increased dependence on smartphones and electronic devices ( OR=1.86, 95%CI =1.32-2.61) and high alienation score (medium level: OR=2.06, 95%CI =1.45-2.92; high level: OR=5.19, 95%CI =1.00-27.00) were positively correlated with social anxiety. Increased communication with friends ( OR=0.65, 95%CI =0.47-0.90 and high self efficacy (medium level: OR= 0.37 , 95%CI =0.21-0.66; high level: OR=0.15, 95%CI =0.08-0.30) were negatively correlated with social anxiety.@*Conclusion@#At the normalization stage of COVID-19 epidemic prevention and control, social anxiety of graduate students is one of the mental health issues which need further attention. Participation in peer support helps prevent social anxiety through developing self efficacy, alleviating individual alienation, and reducing dependence on electronic devices among graduate students.

ABSTRACT

Objective:To investigate the effects of Ureaplasma urealyticum GrpE ( Uu-GrpE) on the maturation of dendritic cells and the polarization of T cells. Methods:Uu-GrpE was expressed and purified, and then identified by Western blot. The cytotoxicity of Uu-GrpE to mouse bone marrow-derived dendritic cells (BMDCs) was analyzed by LDH kit. After stimulating BMDCs with Uu-GrpE, the expression of costimulatory molecules, CD80, CD86 and major histocompatibility complex Ⅱ (MHCⅡ), on the surface of BMDCs was detected by flow cytometry, and ELISA was used to detect the cytokines such as IL-12p70, TNF-α, IL-1β and IL-6. CD4 + Na?ve T cells were isolated from mouse spleen tissues by magnetic beads. A co-culture system of BMDCs and Na?ve T cells was constructed to analyze the effects of GrpE-stimulated mature BMDCs (GrpE-BMDCs) on T cell proliferation and polarization towards Th1/Th2. Mice were immunized with GrpE-BMDCs through the tail vein, and the induced humoral and cellular immune responses were detected by ELISA and flow cytometry. Results:Uu-GrpE was successfully express and high purity BMDCs were isolated. Uu-GrpE could stimulate BMDCs to secrete cytokines such as IL-12p70, TNF-α, IL-1β and IL-6 without having cytotoxicity. Uu-GrpE significantly increased the expression of CD80 [mean flourscence indensity (MFI): (324.00±22.11) vs (91.03±10.95), P<0.01], CD86 [MFI: (1 176.00±51.39) vs (217.00±14.93), P<0.01] and MHCⅡ [MFI: (708.70±56.32) vs (185.70±16.77), P<0.01] on BMDCs. Compared to the GrpE-BMDCs only group and GrpE (boiled)-BMDCs+ T cell group, the GrpE-BMDCs+ T cell group showed significantly increased T cell proliferation [stimulation index: (7.25±0.21) vs(6.55±0.23) and (6.09±0.35), both P<0.05], and dramatically promoted T cell secretion of IL-2 and IFN-γ [IL-2: (145.60±14.67) pg/ml vs(55.92±3.12) pg/ml and (26.05±2.40) pg/ml, P<0.05 and P<0.01; IFN-γ: (267.20±37.80) pg/ml vs(146.70±20.65) pg/ml and(27.84±6.69) pg/ml, both P<0.05]. However, no significant change was observed in the expression of Th2-type cytokines. Moreover, the adoptive transfer of GrpE-BMDCs induced a Th1-type immune response. Conclusions:Uu-GrpE could stimulate the maturation and polarization of BMDCs. Moreover, it could induce Th1 immune response as a candidate protein vaccine for Ureaplasma urealyticum.

ABSTRACT

Objective:To discuss the application and effects of blended learning (B-Learning) mode in medical courses and education at home and abroad.Methods:Based on the identified keywords, the research retrieved 93 relevant literature from 2000 to 2017 in journal databases (e.g. Premier, Cinahl and Medline), and published journals. After two rounds of analysis, 19 domestic and foreign literature were obtained, which evaluated the application and teaching effects of the B-Learning mode in the context of medical education. Among them, there were control studies and qualitative studies.Results:There are differences in the implementation and implementation of the B-Learning model in various countries; the teaching intervention measures and the scope of intervention of the B-Learning teaching model are diverse; the B-Learning model can make up for the gap between the theoretical and practical courses in the medical education process. It helps medical students to master their knowledge and improve their clinical skills; it is necessary for teachers to carefully design teaching courses and teaching research before class to improve the teaching effect of B-Learning.Conclusion:Combining modern educational technology with traditional teaching methods can effectively improve the medical knowledge level and clinical practice skills of medical students.

ABSTRACT

Radiotherapy is an important means to treat malignant tumors in clinical practice. However, tumor radioresistance not only seriously limits the efficiency of radiotherapy, but also leads to poor prognosis of patients, which may eventually lead to tumor metastasis and recurrence. Circular RNA (circRNA) is a new member of noncoding RNA, which is highly specific, stable and conservative. This paper reviewed with the effect of circRNA on the signal pathways related to radiotherapy resistance by analyzing the perspective of DNA damage repair pathway, EMT (epithelial mesenchymal transition) process, Wnt pathway, tumor stem cells and other pathways, and sorted out the mechanism of circRNA in tumor cell resistance to radiotherapy, which may provide valuable clues for further study of circRNA.

ABSTRACT

Objective:To observe the effect of Jinxiangdan (JXD) on NOD-like receptor pyrin domain-containing-3 (NLRP3)/cysteine-dependent aspartate-directed protease-1 (Caspase-1)/interleukin-1<italic>β</italic> (IL-1<italic>β</italic>) signaling pathway in myocardium of rats with myocardial ischemia-reperfusion injury (MIRI) and explore the protective effect and mechanism of JXD against MIRI. Method:Fifty male SD rats were randomly divided into the sham operation group, model group, high- and low-dose JXD groups, and positive drug (Di'ao Xinxuekang) group, with 10 rats in each group. Seven days before modeling, rats in the JXD groups were separately treated with intragastric administration of 0.72 and 0.18 g·kg<sup>-1</sup> JXD tablets, the ones in the sham operation group and model group with the same volume of normal saline, and those in the positive drug group with 1.29 g·kg<sup>-1</sup> Di'ao Xinxuekang. Twelve hours after the last intragastric administration, the anterior descending branch of the left coronary artery was ligated for 30 min and then re-perfused for 60 min for inducing MIRI. ST segment elevation was detected by electrocardiogram(ECG) for model evaluation. The contents of creatine kinase (CK) and lactate dehydrogenase (LDH) in cardiac tissue were measured by colorimetry. Hematoxylin-eosin (HE) staining was conducted for observing myocardial histopathological changes, followed by the detection of cardiomyocyte apoptosis by DNA in situ nick end-labeling (TUNEL) assay. The protein and mRNA expression levels of NLRP3, Caspase-1, and IL-1<italic>β</italic> were detected by Western blot and real-time polymerase chain reaction (Real-time PCR), respectively. Result:Compared with sham operation group, the model group exhibited obviously elevated ST segment (<italic>P</italic><0.01), enhanced CK and LDH activities in the myocardium (<italic>P</italic><0.01), increased apoptotic cardiomyocytes (<italic>P</italic><0.01), and up-regulated NLRP3, Caspase-1, and IL-1<italic>β</italic> protein and mRNA expression (<italic>P</italic><0.01). Compared with model group, JXD at both the high and low doses and Di'ao Xinxuekang significantly lowered the ST segment (<italic>P</italic><0.05,<italic>P</italic><0.01), diminished the CK and LDH activities in myocardial tissue (<italic>P</italic><0.05,<italic>P</italic><0.01), improved the apoptosis of cardiomyocytes (<italic>P</italic><0.05,<italic>P</italic><0.01), and down-regulated the mRNA and protein expression levels of NLRP3, Caspase-1, and IL-1<italic>β</italic> in myocardial tissue (<italic>P</italic><0.05,<italic>P</italic><0.01). The ST segment of ECG in the low-dose JXD group was increased as compared with that in the Di'ao Xinxuekang group (<italic>P</italic><0.05), while the ST segment in the high-dose JXD group was obviously elevated (<italic>P</italic><0.05). Besides, the green fluorescence intensities in the low- and high-dose JXD groups and the Di'ao Xinxuekang group remarkably declined (<italic>P</italic><0.05,<italic>P</italic><0.01). The mRNA and protein expression levels of NLRP3, Caspase-1, and IL-1<italic>β</italic> in the high-dose JXD group were down-regulated (<italic>P</italic><0.05). Conclusion:JXD alleviates MIRI possibly by lowering NLRP3 and IL-1<italic>β</italic> expression and inhibiting cardiomyocyte apoptosis.

OBJECTIVES@#To study the effect of gap junction blockers, quinine (QUIN) and carbenoxolone (CBX), on hippocampal ripple energy expression in rats with status epilepticus (SE).@*METHODS@#A total of 24 rats were randomly divided into four groups: model, QUIN, valproic acid (VPA), and CBX (@*RESULTS@#Ripple expression was observed in the hippocampal CA1, CA3, and dentate gyrus regions of normal rats. After 10 minutes of PILO injection, all groups had a gradual increase in mean ripple energy expression compared with 1 day before modeling, with the highest expression level before chloral hydrate injection in the model, VPA and CBX groups (@*CONCLUSIONS@#The change in ripple energy can be used as a quantitative indicator for early warning of seizures, while it cannot predict seizures in the interictal period. Gap junction blockers can reduce ripple energy during seizures.

ABSTRACT

Objective:To understand the current situation of human resources in the hemodialysis centers of different levels of hospitals in Beijing, and to provide reference data for themedical institutions at all levels to adjust the related human resources programs.Methods:Based on the self-designed questionnaire, the basic situation of 123 hemodialysis centers and the allocation of human resources of doctors, nurses and engineers in medical institutions of Beijing were investigated and analyzed.Results:Each nurse in hemodialysis center was responsible for (4.98± 0.74) hemodialysis patients at the same time in each treatment shift. The specialized nurses accounted for 23.32% of the total registered nurses. The ratio of engineers to dialysis machines was 1∶25.76. The proportion of doctors, nurses and engineers with intermediate professional titles or above was 82.47%, 33.08% and 29.81% respectively. The proportion of doctors with graduate education or above was 51.13%. The proportion of nurses and engineers with bachelor degree or above was 56.12% and 51.56% respectively. The differences in the professional titles and the highest academic qualifications of the employees (doctors, nurses and engineers) between tertiary hospitals and non-tertiary hospitals were statistically significant ( P < 0.05). Conclusion:The number of personnel in Beijing hemodialysis centers is basically in a similar proportion, but there are some differences between the hemodialysis centers of tertiary hospitals and those of non-tertiary hospitals in terms of age composition, working years, professional titles and educational background. It is necessary to optimize the allocation of human resources in hemodialysis centers of different levels of hospitals, further strengthen the training of professional talents and the cooperation of multidisciplinary teams, to promote the sustainable development of medical service quality.

ABSTRACT

Objective:To apply the B-learning teaching mode into the Medical Statistic course, and to comparatively analyze the students' knowledge, attitude, and practice (KAP). Methods:In this study, undergraduates were investigated by self-designed questionnaires, and the reliability and validity of the questionnaire were evaluated. SAS 9.3 statistical software was used to analyze the KAP evaluation and the influencing factors of online learning with chi-square test, t test and multiple regression analysis. Results:There were significant differences in students' cognitive status in three aspects: the role of online videos ( P=0.012 9), whether to submit assignments ( P=0.040 7), and whether to participate in discussions ( P=0.009 4). In terms of attitude, there were significant differences between the role of B-learning process ( P=0.027 1) and online learning interest ( P=0.013 7). And in the terms of practice, there were significant differences in the time of online learning ( P=0.002 4), weekly login times for online learning ( P=0.015 4) and ways to solve difficulties ( P=0.004 5). Conclusion:The B-learning mode applied to the course of Medical Statistics has greatly improved students' ability in knowledge, attitude and practice (KAP), and achieved good teaching effects.

ABSTRACT

OBJECTIVE:To evaluate the efficacy and safety of prophylactic use of low-dose aspirin in patients with chronic renal disease ,and to provide reference for evidence-based reference for rational use in the clinic. METHODS :Retrieved from PubMed,Embase,Cochrane Library ,CNKI,VIP,Wanfang database ,RCTs about prophylactic use aspirin 75-150 mg/d(aspirin group)versus no aspirin and other antiplatelet drugs or placebo (control group )were collected from inception to May 14th,2019. Relevant data were extracted. After quality evaluation with bias risk assessment tool recommended by Cochrane system evaluator manual 5.1.0,Meta-analysis of the incidence of cardiovascular events ,renal events and bleeding events in patients with chronic renal disease was performed by using Rev Man 5.3 software. RESULTS :A total of 5 RCTs were included ,involving 4 728 patients;among them ,there were 2 392 persons in aspirin group and 2 390 persons in control group. Results of Meta-analysis showed that there was no statistical significance in the incidence of cardiovascular events [OR =0.59,95%CI(0.13,1.12),P=0.11] and the incidence of renal events [OR =0.42,95%CI(0.08,2.23),P=0.31] between aspirin group and control group. The incidence of bleeding events [OR =2.12,95%CI(1.23,3.66),P=0.007] in aspirin group was significantly higher than control group. After exculsion of two heterogeneity literatures ,the incidence of cardiovascular events [OR =0.27,95%CI(0.13,0.55),P=0.000 3] in aspirin group was significantly lower than control group. CONCLUSIONS :Prophylactic use of low-dose aspirin can prevent cardiovascular events in patients with chronic renal disease to certain extent. The low-dose aspirin should be used carefully to prevent cardiovascular events in chronic renal disease patients with high risk of bleeding.

OBJECTIVE@#To evaluate the regulation of VEGF-Notch signaling pathway on proliferation and apoptosis of mesenchymal stem cells (MSC) in the patients with aplastic anemia (AA).@*METHODS@#The bone marrow specimens of AA patients were collected for isolation and identification of BM MSC. Westen blot was used to detect the expression of VEGF-Notch signaling pathway-related proteins (VEGF, VEGFR, Notch 1, Jagged 1, Delta-like 1 and Hes1). The VEGF (100 ng/ml) and DAPT (γ-secretase inhibitor, 10 μmol/L) were respectively added into MSC culture system in oder to activate and inhibit the signaling transduction of VEGF-Notch in BM MSC. The proliferation, apoptosis and cell cycle of MSC in AA patients were detected by CCK8 assay and flow cyfometry. The adipogenic differentiation of BM MSC was detected by oil red O staining.@*RESULTS@#The VEGF-Notch signaling pathway was significantly inhibited in AA BM tissues and AA MSC (P<0.05) detected by Western blot. The intervention of VEGF and DAPT significantly activated and inhibited VEGF-Notch signaling in AA MSC, respectively. CCK8 assay showed that VEGF intervention significantly promoted the proliferation of MSC in AA patients (P<0.05). Flow cytometry showed that VEGF significantly inhibited apoptosis of MSCs by blocking S phase cells (P<0.05).@*CONCLUSION@#The activation of VEGF-Notch can restore the proliferation function of MSC in AA patients.

ABSTRACT

OBJECTIVE@#To compare the clinical efficacy between acupuncture combined with cinesiotherapy cupping and acupuncture combined with conventional cupping for knee osteoarthritis (KOA) with stagnation and blood stasis syndrome, and to seek a better solution for KOA.@*METHODS@#A total of 78 patients of KOA with stagnation and blood stasis syndrome were randomly divided into an observation group and a control group, 39 cases in each group (3 cases in the observation group and 2 cases in the control group lost contact). Both groups were treated with acupuncture at Neixiyan (EX-LE 4), Dubi (ST 35), Xuehai (SP 10), Liangqiu (ST 34), Heding (EX-LE 2), Zusanli (ST 36), Yinlingquan (SP 9), Yanglingquan (GB 34) and Xuanzhong (GB 39). Based on the acupuncture treatment, the control group was treated with conventional cupping. The No. 4 cupping glass was used for Xuehai (SP 10), Liangqiu (ST 34) and Fengshi (GB 31), while the No. 3 cupping glass was used for Yinlingquan (SP 9), while the cupping with appropriate size was used for points; the cupping glass was retained for 5 min. Based on the acupuncture treatment, the observation group was treated with cinesiotherapy cupping. The selection of acupoint and cupping glass was identical as the control group. The patients were instructed to perform knee flexion-extension, hip abduction-adduction, weight-bearing and other active exercise while cupping; the treatment was given once a day, 10 times as a course of treatment; totally three courses were given with an interval of 2 days between the courses. The patient's symptom scores, pain scores and knee function scores were recorded before and after treatment. The amount of joint effusion was measured by ultrasound; the level of interleukin-1 (IL-1), interleukin-6 (IL-6), tumor necrosis factor-α (TNF-α) in joint effusion were measured by ELISA.@*RESULTS@#After treatment, the total effective rate in the observation group was 94.4% (34/36), which was significantly higher than 86.5% (32/37) in the control group (0.05).@*CONCLUSION@#The acupuncture combined with cinesiotherapy cupping could alleviate pain, improve joint function and reduce joint effusion, which is superior to acupuncture combined with conventional cupping.

ABSTRACT

Objective To evaluate the efficacy of dual-wavelength non-stripping dot array laser " water enhancement" in the treatment of facial sunken scar.Methods From June 2017 to February 2018,fifty patients with facial sunken scar were randomly divided into observation group and control group according to the digital table,with 25 cases in each group.The observation group was treated by lidocaine injection combined with dual-wavelength non-stripping lattice laser,while the control group was treated with double-wavelength non-stripping dot array laser. The clinical therapeutic effects of the two groups were compared and analyzed.Results The cure rate of the observa-tion group was 60.00% (15/25),which was significantly higher than that of the control group[36.00% (9/25)], and the difference between the two groups was statistically significant (χ2 =32.524,P<0.05).Conclusion Water assisted 1540 nm lattice laser therapy for facial sunken scar after acne has obviously better effect,safety and less adverse reactions.And it is the most suitable treatment for patients with facial sunken scar after acne in Asia.

ABSTRACT

Objective To investigate the application of individualized health education in patients with diabetes and its effect on compliance with doctor's advice.Methods A total of 102 patients with diabetes who were admitted to our hospital were randomly divided into control group and observation group,with 51 patients in each group.The patients in the control group were given conventional hypoglycemic therapy,and those in the observation group were given hypoglycemic therapy combined with individualized health education.Health education investigation was performed to evaluate patients' compliance with doctor's advice.SPSS 13.0 was used for the chi-square test.Results The observation group had a significantly higher rate of compliance with doctor's advice than the control group from the following five aspects:treatment compliance (98.04% vs 80.39%,P<0.05),smoking quitting and alcohol limiting (92.16% vs 72.55%,P<0.05),rational diet (86.27% vs 66.67%,P<0.05),exercise (80.39% vs 58.82%,P<0.05),and blood glucose moritoring (84.31% vs 62.75%,P<0.05).Conclusion The application of individualized health education in patients with diabetes can effectively improve their compliance with doctor's advice,and therefore,it holds promise for clinical application.

@#【Objective】To evaluate the effect of preoperative conjunctival sac irrigation with Anerdian Ⅲ disinfectant and its effect on the function of eye surface and tear film. 【Methods】 This clinical study was involved of thirty cases (30 eyes) undergoing phacoemulsification. Conjunctiva sac irrigation with Anerdian disinfectant (type III ,5g/L) was performed before the phacoemulsification procedure. Secretions of conjunctiva sac were examined by bacterial culture pre-and post-irrigation. Schirmer test,non-invasive first tear break-up time(NITBUT-F),non-invasive average tear break-up time(NITBUT-A),tear meniscus height(TMH),and goblet cell density(GCD)were also evaluated day 1 before and day 7 ,day 30 after operation.【Results】3 cases were positive in bacterial culture pre-irrigation ,while all were negative post-irrigation. There was no significantly change between Schirmer test and TMH from day 1 before ,day 7 and day 30 after operation . NITBUT-F,NITBUT-A,and GCD were significantly different between day 1 before and day 7 after operation(P<0.05),and shown no significantly difference between day 1 before and day 30 after operation.【Conclusions】 Our results suggested that conjunctiva sac irrigation with Anerdian disinfectant was efficient,while it might be related to the decrease of GCD and dysfunction of tear film. However,GCD would gradually improve as pre-operation and function of tear film could restore to normal.

@#【Objective】To detect the ability and the efficiency of recombinant PTD-HSP27 transport across RGC-5 cells and measure the role of recombinant PTD- HSP27 against oxidative stress damage induced by cobalt chloride on RGC- 5 cells,and explored its potential mechanism tentatively. 【Methods】 RGC- 5 cells were incubated with PTD- HSP27 labeled with FITC,followed by the observation of fluorescence using fluorescence microscope. We also lysed the cells by radio immunoprecipitation assay and measure the transport efficiency of PTD- HSP27- FITC through ultraviolet spectrophotometer subsequently. We established the RGC- 5 cell damage model. Cellular experiments were divided into three groups:normal control group,cobalt chloride damage group,PTD-HSP27 + cobalt chloride treatment group. Those three groups of cells were experimented using Annexin V- FITC/PI staining kit:to detect apoptotic cell ratio;Western Blotting:to detect the expression level of apoptosis related protein,including Bcl-2,Bax and Caspase-3.【Results】The fluorescence of PTD- HSP27- FITC was visualized inside the RGC- 5 cells using the fluorescence microscope,while the transport efficiency of PTD-HSP27-FITC was(47.29 ± 2.33)%. Cobalt chloride inhibited the survival vitality of RGC- 5 cells in a concentration- dependent manner. The comparison among normal control group,cobalt chloride damage group and PTD-HSP27+CoCl2 treatment group showed:PTD-HSP27 could effectively suppressed early apoptosis induced by cobalt chloride(P<0.01). Western Blotting results showed that PTD-HSP27 could effectively enhance the expression ratio of Bcl-2/Bax,and suppressed the activation of Caspase- 3(P<0.01).【Conclusions】Recombinant protein PTD- HSP27 could protect RGC- 5 cells against oxidative stress injury induced by cobalt chloride. Iit mainly regulated the expression ratio of Bcl-2/Bax,suppressed early apoptosis and improved cell viability in RGC-5 cells.

ABSTRACT

Objective To investigate the effect of hepatitis B virus(HBV)infection on serum iron markers in pregnant women.Methods A total of 188 pregnant women with HBV infection and 157 normal pregnant women were recruited in this study.Serum levels of iron,ferritin,transferrin,HBV DNA,alanine aminotrans-ferase(ALT)and aspartate aminotransferase(AST)were measured and compared.Results At the same stage of pregnancy,the serum ferritin level in pregnant women with HBV infection was higher than that of normal pregnant women(Z= -1.72,P=0.04;Z= -2.33,P=0.01;Z= -4.42,P=0.01),while the serum transfer-rin concentration increased in normal pregnant women in the second and third trimesters,and the differences between the two groups were statistically significant(Z= -3.26,P<0.01;Z= -2.25,P=0.01).In pregnant women with HBV infection,the serum ferritin levels in patients with positive results of ALT and AST were higher than those in patients with negative results(ALT:P=0.01;AST:P=0.02),however,there was no change in healthy pregnant women.Conclusion There is iron metabolism imbalance in pregnant women with HBV infection.We should monitor serum iron markers to guide pregnant women for appropriate iron supple-mentation in gestation period.

ABSTRACT

ATP-sensitive potassium channels (K) are energy sensors on the plasma membrane. By sensing the intracellular ADP/ATP ratio of β-cells, pancreatic K channels control insulin release and regulate metabolism at the whole body level. They are implicated in many metabolic disorders and diseases and are therefore important drug targets. Here, we present three structures of pancreatic K channels solved by cryo-electron microscopy (cryo-EM), at resolutions ranging from 4.1 to 4.5 Å. These structures depict the binding site of the antidiabetic drug glibenclamide, indicate how Kir6.2 (inward-rectifying potassium channel 6.2) N-terminus participates in the coupling between the peripheral SUR1 (sulfonylurea receptor 1) subunit and the central Kir6.2 channel, reveal the binding mode of activating nucleotides, and suggest the mechanism of how Mg-ADP binding on nucleotide binding domains (NBDs) drives a conformational change of the SUR1 subunit.

ABSTRACT

Objective To investigate the clinical diagnosis features of primary pulmonary lymphoma (PPL). Methods 11 cases of primary pulmonary lymphoma, confirmed by pathological examinations from January 2015 to March 2017, were studied. Most of them (10 cases) were diagnosed by endobronchial ultrasound. Results The study group consisted of 7 male (63.6%) and 4 female (36.4%) patients, aged from 35 to 72 years. 8 of 11 cases (72.7%) were more than 50 years old. The clinical symptoms consist of cough (63.6%), sputum (45.5%), breathlessness(36.4%), 2 cases were asymptomatic. The pulmonary imaging manifestation consists of four different patterns: pneumonic (72.7%);nodular or mass (45.5%); bronchovascular-lymphangitic (9.1%); milliary- hematogenous (9.1%). Air bronchograms (8 cases, 72.7%) were the common accompanying signs, typically taking the shape of withered arborization. Many cases (72.7%) were misdiagnosed, first as pneumonia or organizing pneumonia. Conclusion Primary lymphoma of the lung occurs with nonspecific clinical features, often initial misdiagnosed as pneumonia or organizing pneumonia, but it has some characteristics on radiology, such as multiple lesions, lesions accompanied with air bronchogram taking the shape of withered arborization. Final diagnosis making depended on pathological examinations by lung biopsy. The positive rate of lung biopsy by endobronchial ultrasound is also high.
